A $100 cash award will be issued to all employees and spouses upon completion of the first and second incentive steps by October 31, 2012: an online Health Risk Assessment (HRA) and the Eat Right for Life Guide. In addition, a complimentary copy of the the Eat Right for Life cookbook will be sent via interoffice mail to all participants who complete these two steps. Incentive checks will be mailed monthly beginning in late February and are limited to one $100 check per participant per year for these two steps.

A second $100 cash award will be issued to employees and spouses upon completing all three incentive steps by October 31, 2012: one Health Risk Assessment, the Eat Right for Life guide and one 12-week fitness program. Beginning in late April, $100 awards will be issued monthly and are limited to one completion check per participant per year.  

Participants who complete a Health Risk Assessment will receive ONE ENTRY into the bi-monthly drawing* for EACH Health Education Program completed between January 1, 2012 and October 31, 2012.

>March: (1) iPad and (10) $100 gift cards
>May: (1) iPad and (20) $100 gift cards
>July: (1) iPad and (30) $100 gift cards
>Sept: (1) iPad and (40) $100 gift cards
>Nov: (1) iPad and (15) $250 gift cards 
Winners will be announced in NUToday the first Monday of each month.


You must be an eligible employee or spouse of an eligible employee on the date the award is mailed to receive the award. The last day to inquire about 2011 awards due is Dec. 30, 2011. After that day, only inquiries about the 2012 program awards will be addressed.
