Get Started: Health Risk Assessment

Participants must complete a confidential Health Risk Assessment (HRA) in 2012. Both the employee and spouse must use the employee's Employee ID Number to access the online assessment. This number is a four or five digit number which can be found on the employee's badge under the bar code or in the NUNet phone book under employee name.

Upon completion of the HRA, a copy of the Eat Right for Life guide will automatically be sent through the interoffice mail to the employee or employee's spouse. This guide is needed to complete Step 2: Get Nourished.

Online Health Risk Assessment and Results

For your HRA to be processed, you will be required to include your first and last name, employee ID number, gender and date of birth. Other biometric data such as blood pressure, cholesterol, triglycerides and blood sugar will enhance your scope of results but are not required.

Please note: You are not required to complete the family history section of the HRA in order to obtain the incentive awards offered through the WellAware program.

University of Michigan Health Resource Management Center, an independent company that has specialized in health and wellness programs for more than 20 years, will analyze your responses and prepare your personal HRA results.
